Roles & Responsibilities

• Evaluate patients in their homes and create individualized, evidence-based plans of care
• Deliver skilled physical therapy interventions and modify treatment as needed
• Master and accurately score CMS OASIS assessments and collaborate with Quality teams for adjustments
• Document point-of-care treatments, communications, and adverse events thoroughly
• Coordinate care with physicians, nurses, and other disciplines to ensure safe, effective plans
• Supervise PTAs and CNAs when involved in patient care and perform required supervisory visits
• Participate in quality assurance and performance improvement initiatives to support agency goals
• Manage productivity expectations while maximizing continuity of care and efficient travel routes
• Maintain accurate time logs and mileage records following agency policies

Qualifications

• Current Montana Physical Therapy license (required)
• BS degree or higher from an accredited Physical Therapy program (BS, MS, or Doctorate acceptable)
• Valid driver's license and appropriate auto insurance
• Two years of PT experience preferred
• Comfortable performing interventions allowed under the PT practice act, including wound care
• Strong ability to work independently in community/home settings

Education

• BS in Physical Therapy or higher (MS or Doctorate of Physical Therapy preferred)

Knowledge & Skills

• Skilled in patient evaluation, plan of care development, and rehabilitation interventions
• Proficient with CMS OASIS scoring and Medicare/home health regulations
• Excellent point-of-care documentation and clinical decision-making
• Experience with wound care and supervision of ancillary staff (PTAs/CNAs)
• Strong patient and family education and interdisciplinary communication skills
• Familiarity with care coordination, quality assurance processes, and productivity management
• Ability to plan efficient travel routes and manage time/mileage logging
