Roles & Responsibilities
- Evaluate patients in their homes and create individualized, evidence-based plans of care
- Deliver skilled physical therapy interventions and modify treatment as needed
- Master and accurately score CMS OASIS assessments and collaborate with Quality teams for adjustments
- Document point-of-care treatments, communications, and adverse events thoroughly
- Coordinate care with physicians, nurses, and other disciplines to ensure safe, effective plans
- Supervise PTAs and CNAs when involved in patient care and perform required supervisory visits
- Participate in quality assurance and performance improvement initiatives to support agency goals
- Manage productivity expectations while maximizing continuity of care and efficient travel routes
- Maintain accurate time logs and mileage records following agency policies
Qualifications
- Current Montana Physical Therapy license (required)
- BS degree or higher from an accredited Physical Therapy program (BS, MS, or Doctorate acceptable)
- Valid driver's license and appropriate auto insurance
- Two years of PT experience preferred
- Comfortable performing interventions allowed under the PT practice act, including wound care
- Strong ability to work independently in community/home settings
Education
- BS in Physical Therapy or higher (MS or Doctorate of Physical Therapy preferred)
Knowledge & Skills
- Skilled in patient evaluation, plan of care development, and rehabilitation interventions
- Proficient with CMS OASIS scoring and Medicare/home health regulations
- Excellent point-of-care documentation and clinical decision-making
- Experience with wound care and supervision of ancillary staff (PTAs/CNAs)
- Strong patient and family education and interdisciplinary communication skills
- Familiarity with care coordination, quality assurance processes, and productivity management
- Ability to plan efficient travel routes and manage time/mileage logging
