Roles & Responsibilities

• Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse in an acute/long-term care setting
• Administer medications and monitor patient responses accurately and safely
• Perform routine assessments and promptly report changes in condition to the care team
• Document care clearly and thoroughly in the electronic health record to ensure continuity
• Assist patients with activities of daily living and promote dignity and independence
• Collaborate with RN colleagues, physicians, and interdisciplinary team members to deliver coordinated care
• Adhere to infection control standards and facility policies to maintain a safe environment
• Support smooth transitions of care through effective communication with families and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license valid in New Mexico
• Current Basic Life Support (BLS) certification (preferred or obtained upon hire)
• Clinical experience as an LPN in long-term care, skilled nursing, or similar settings preferred
• Ability to work the scheduled 12+ hour day shifts (early morning start to early evening)
• Reliable, professional, and able to adapt to a fast-paced, team-oriented environment

Education

• Vocational nursing diploma or equivalent (LPN program completion)

Knowledge & Skills

• Strong medication administration and documentation skills
• Comfortable performing routine patient assessments and reporting findings
• Familiarity with infection prevention practices and safety protocols
• Good communication skills for patient, family, and team interactions
• Basic wound care and ADL support experience
• Ability to use electronic health records and follow care plans accurately
