Roles & Responsibilities
- Provide compassionate, patient-centered nursing care as a Licensed Practical Nurse in an acute/long-term care setting
- Administer medications and monitor patient responses accurately and safely
- Perform routine assessments and promptly report changes in condition to the care team
- Document care clearly and thoroughly in the electronic health record to ensure continuity
- Assist patients with activities of daily living and promote dignity and independence
- Collaborate with RN colleagues, physicians, and interdisciplinary team members to deliver coordinated care
- Adhere to infection control standards and facility policies to maintain a safe environment
- Support smooth transitions of care through effective communication with families and staff
Qualifications
- Active Licensed Practical Nurse (LPN) license valid in New Mexico
- Current Basic Life Support (BLS) certification (preferred or obtained upon hire)
- Clinical experience as an LPN in long-term care, skilled nursing, or similar settings preferred
- Ability to work the scheduled 12+ hour day shifts (early morning start to early evening)
- Reliable, professional, and able to adapt to a fast-paced, team-oriented environment
Education
- Vocational nursing diploma or equivalent (LPN program completion)
Knowledge & Skills
- Strong medication administration and documentation skills
- Comfortable performing routine patient assessments and reporting findings
- Familiarity with infection prevention practices and safety protocols
- Good communication skills for patient, family, and team interactions
- Basic wound care and ADL support experience
- Ability to use electronic health records and follow care plans accurately
