Roles & Responsibilities

• Provide compassionate personal care and assistance with activities of daily living (bathing, dressing, toileting) to support patient comfort and dignity
• Measure and record vital signs, intake/output, and other observations accurately and promptly
• Assist nursing staff with patient transfers, mobility, and positioning to promote safety
• Maintain a clean, safe environment and follow infection control protocols to protect patients and colleagues
• Document care delivery clearly and communicate changes in patient condition to the nursing team
• Support a positive patient experience by offering friendly, respectful, person-centered care

Qualifications

• Active CNA certification in Pennsylvania (required)
• Current CPR/BLS certification (preferred)
• Ability to pass background checks, drug screen, and any required health screenings
• Comfortable working in a long-term/rehab care environment and collaborating with a multidisciplinary team
• Strong communication skills and a caring attitude toward patients and families

Education

• High school diploma or equivalent; completion of an approved CNA training program

Knowledge & Skills

• Basic patient care and safe transfer techniques
• Vital signs collection and accurate documentation
• Infection control, hand hygiene, and standard precautions
• Observational skills to notice and report changes in patient status
• Teamwork, time management, and dependable attendance
