Roles & Responsibilities

• Provide compassionate, hands-on care to residents, assisting with activities of daily living
• Monitor and record vital signs, reporting changes to the nursing team promptly
• Support safe transfers and mobility, using proper body mechanics and assistive devices
• Follow individualized care plans and help implement therapeutic activities
• Maintain a clean, safe and respectful environment for residents
• Communicate clearly and professionally with residents, families and the care team
• Document care accurately and timely in resident records
• Escalate clinical concerns to licensed nurses to ensure timely intervention

Qualifications

• Active CNA certification in Pennsylvania or ability to obtain state certification
• Current basic immunizations and ability to pass required health screenings
• Background check and any facility-required clearances
• Comfort working night shifts and collaborating with an interdisciplinary team
• Previous long-term care or skilled nursing experience preferred

Education

• High school diploma or GED
• Completion of an approved CNA training program and active CNA certification

Knowledge & Skills

• Strong patient care skills including bathing, dressing, feeding and toileting assistance
• Ability to take and accurately record vital signs
• Knowledge of infection control and safe patient handling techniques
• Good communication and interpersonal skills with residents and team members
• Basic charting and documentation skills
• Reliable time management and the ability to prioritize tasks during busy shifts
