Roles & Responsibilities

• Provide compassionate, hands-on care to residents — helping with activities of daily living like bathing, dressing, and grooming
• Monitor and record vital signs and any changes in resident condition, reporting concerns to the nurse in charge
• Assist with safe transfers and mobility to promote independence while preventing falls
• Maintain accurate, timely documentation to support high-quality, coordinated care
• Support infection control practices and maintain a clean, safe environment for residents
• Communicate clearly and kindly with residents and families to build trust and improve outcomes
• Collaborate with the nursing team to implement care plans and ensure each resident’s needs are met

Qualifications

• Current Certified Nursing Assistant (CNA) certification — Pennsylvania certification preferred
• Active placement on the state CNA registry (or eligible for registry) required
• Ability to pass background check and required health screenings
• Previous experience in long-term care or similar clinical setting is a plus
• CPR/BLS certification is a plus and may be requested by the facility
• Strong communication skills and a patient-centered attitude

Education

• High school diploma or GED preferred
• Completion of an accredited CNA training program required

Knowledge & Skills

• Hands-on resident care and assistance with ADLs
• Basic vital sign measurement and observation skills
• Familiarity with infection control procedures and resident safety practices
• Clear, compassionate communication with residents, families, and care teams
• Basic documentation and charting skills
• Ability to follow care plans and take direction from nursing staff
