Roles & Responsibilities

• Provide compassionate direct patient care and assist residents with activities of daily living
• Monitor and record patient vital signs and changes in condition accurately
• Support patient mobility, safe transfers, and fall prevention efforts
• Communicate effectively with nurses, therapists, and families to coordinate care
• Maintain a clean, safe environment and adhere to infection control practices
• Document care in the chart clearly and timely to support care continuity
• Assist with feeding, bathing, grooming, and linen changes while preserving dignity
• Contribute to a positive, person-centered atmosphere that supports patient well-being

Qualifications

• Current Certified Nursing Assistant (CNA) certification (state requirements must be met)
• Active basic life support (BLS/CPR) certification preferred or required depending on facility policy
• Previous experience working as a CNA or in direct patient care is a plus
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication skills and the ability to work collaboratively with care teams

Education

• High school diploma or GED required
• Current CNA certification (state-specific)

Knowledge & Skills

• Knowledge of activities of daily living (ADLs) and basic patient care techniques
• Familiarity with infection control practices and patient safety principles
• Basic charting skills and comfort with electronic or paper documentation
• Strong observation skills and the ability to escalate clinical concerns appropriately
• Good time management, teamwork, and a professional bedside manner
