Roles & Responsibilities

• Assist residents with activities of daily living, including bathing, dressing, and toileting, with dignity and respect
• Take and record vital signs and report any changes in condition to the nurse
• Provide compassionate, hands-on care while building trusting relationships with residents
• Document care accurately and promptly in the resident chart
• Support safe mobility and fall prevention, using proper body mechanics and assistive devices
• Maintain a clean, safe environment by following infection control and facility procedures
• Communicate effectively with the care team and families to ensure continuity of care
• Participate in care planning and follow-through to help residents meet their goals

Qualifications

• Current Certified Nursing Assistant (CNA) certification, or ability to obtain Pennsylvania CNA certification
• Current CPR/BLS certification preferred
• Valid state-required immunizations and ability to pass background and drug screening
• Compassionate patient focus, strong communication skills, and dependable attendance
• Ability to perform physical tasks required for resident care, including lifting and standing for shifts

Education

• High school diploma or GED preferred
• Current CNA certification required

Knowledge & Skills

• Hands-on resident care and ADL assistance
• Basic vital sign measurement and observation skills
• Infection control and safe patient handling practices
• Accurate documentation and basic charting abilities
• Strong teamwork, time management, and interpersonal skills
• Ability to follow care plans and communicate changes to nursing staff
