Roles & Responsibilities

• Provide compassionate direct care to residents—assist with activities of daily living like bathing, dressing, grooming and toileting
• Assist with safe transfers and ambulation to support mobility and reduce fall risk
• Monitor and record vital signs and report changes to nursing staff promptly
• Document care accurately and timely in resident charts to maintain continuity of care
• Support mealtime and dietary needs, including feeding assistance when required
• Maintain infection control practices and a clean, safe environment for residents
• Communicate clearly and respectfully with residents, families, and the care team to promote person-centered care

Qualifications

• Current CNA certification in good standing (or eligible to obtain state certification)
• Active state CNA credential preferred — must meet Pennsylvania requirements if working on-site
• Basic Life Support (BLS) certification preferred or willingness to obtain
• Ability to pass background checks and health screenings (immunizations, TB test) as required by facility
• Comfortable working in long-term/post-acute care environments and collaborating with interdisciplinary teams

Education

• High school diploma or GED preferred
• Certified Nursing Assistant (CNA) credential

Knowledge & Skills

• Hands-on patient care: bathing, grooming, feeding, toileting, mobility assistance
• Measuring and recording vital signs accurately
• Safe transfer and positioning techniques
• Infection prevention and proper PPE use
• Clear communication and strong teamwork skills
• Basic charting and documentation practices
