Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Evaluate patients’ mobility, functional status, and rehabilitation needs in both inpatient and swing-bed settings
- Develop individualized treatment plans that support recovery after orthopedic procedures and medical illness
- Provide skilled outpatient orthopedics care, and apply neurorehabilitation techniques when appropriate
- Deliver compassionate hands-on therapy, progressive exercise programs, and safe mobility training
- Collaborate with nursing, case management, and caregivers to coordinate timely discharges and home readiness
- Document clinical progress clearly and on time using facility-required processes and checklists
- Participate in Trinity Specialty Specific Competency Testing and complete the Trinity Skills Checklist to ensure consistent, high-quality care
Qualifications
- Licensed Physical Therapist (must have, or be eligible to obtain Oregon license prior to start)
- BLS certification (required)
- Willingness to complete Trinity competency testing and skills checklist as part of onboarding
- Ability to pass required health screenings and pre-employment testing (MMR, Hep B, PPD/IGRA, Varicella, COVID-19, flu, physical, 10-panel drug screen)
- Comfort working in a critical access rural hospital environment with a mix of inpatient, swing bed, and outpatient caseloads
- Experience in inpatient rehabilitation, swing bed rehab, outpatient orthopedics, or neurorehabilitation is highly desirable
Knowledge & Skills
- Strong assessment and treatment planning skills for orthopedic and medical rehabilitation
- Hands-on manual therapy and therapeutic exercise prescription
- Experience with mobility training, transfers, gait training, and assistive device recommendations
- Familiarity with discharge planning and caregiver education in a rural hospital setting
- Good communication and interdisciplinary collaboration skills
- Ability to adapt treatment approaches across inpatient, swing-bed, and outpatient settings
