Roles & Responsibilities

• Provide compassionate bedside care to residents, ensuring dignity and comfort
• Administer medications and treatments accurately and on schedule
• Perform thorough patient assessments and document changes in condition promptly
• Collaborate with the care team to develop and follow individualized care plans
• Assist with activities of daily living and promote resident independence whenever possible
• Maintain clear, timely documentation in the resident record and communicate effectively with families
• Adhere to infection control standards and facility policies to keep patients safe
• Support onboarding and mentorship for newer staff when appropriate

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania required
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification preferred or willingness to obtain
• Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong communication skills and the ability to work well within a multidisciplinary team
• Reliable, professional, and patient-centered approach to care

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program
• Current LPN licensure in Pennsylvania

Knowledge & Skills

• Medication administration and documentation best practices
• Resident assessment and monitoring for common long-term care conditions
• Wound care basics and pressure injury prevention
• Comfortable with electronic medical records and timely charting
• Effective communication with residents, families, and clinical staff
• Time management and prioritization during busy shifts
