Roles & Responsibilities
- Provide compassionate direct patient care to residents with dignity and respect
- Administer medications and treatments accurately and document per policy
- Monitor and record vital signs, reporting changes to the care team promptly
- Assist residents with activities of daily living and mobility needs
- Collaborate with nurses, therapists, and support staff to deliver coordinated care
- Follow infection control, safety protocols, and facility procedures consistently
- Communicate clearly with residents and families to build trust and reassurance
Qualifications
- Valid Licensed Practical Nurse (LPN) license (required)
- Current CPR/BLS certification (or willing to obtain)
- Comfortable working in a long-term care / skilled nursing environment
- Strong communication skills and a team-first attitude
- Reliable, compassionate, and detail-oriented — medication and documentation skills
Education
- Practical Nursing diploma or equivalent completion of an accredited LPN program
Knowledge & Skills
- Medication administration and basic clinical assessments
- Wound care fundamentals and infection control practices
- Electronic medical record (EMR) documentation and charting
- Resident-centered care and ADL support
- Time management, prioritization, and teamwork
