Shift Type: Monday-Friday, 8:30 AM - 5:00 PM Day (5x8) with periodic weekend coverage required
Hours per Week: 40
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Conduct thorough psychosocial and biopsychosocial assessments to identify patient and family needs
- Develop and implement person-centered discharge plans in collaboration with the interdisciplinary team
- Provide brief clinical interventions, suicide screening, and safety planning as indicated
- Coordinate community resources and referrals to support safe transitions of care
- Document assessments, plans, interventions, and responses clearly in the medical record (Epic)
- Work closely with RN case managers and other disciplines to facilitate continuity of care
- Complete required reports of harm per state statutes and follow mandated reporting procedures
- Float within scope to meet facility needs across Emergency, Children’s, and Adult teams
Qualifications
- MSW from a CSWE-accredited program preferred; LCSW or LMSW accepted
- AK license as LMSW or LCSW required (pending AK license accepted)
- Minimum 1 year of professional experience required; 2 years preferred, ideally with acute care experience
- Comfortable with inpatient acute care settings including ED, ICU, medical-surgical and pediatrics
- Experience with Epic charting preferred
- Strong assessment, crisis intervention, safety planning, and discharge planning skills
- Ability to collaborate effectively with interdisciplinary teams and maintain professional documentation
Education
- Master of Social Work (MSW) from a CSWE-accredited school
- LMSW or LCSW licensure (Alaska) — pending license acceptable during onboarding
Knowledge & Skills
- Case management, discharge planning, and care coordination expertise
- Biopsychosocial assessment, MMSE and geriatric assessment familiarity
- Suicide screening, safety planning, and high-risk intervention skills
- Knowledge of community resources: Medicaid, Medicare, transportation, housing, and social supports
- Experience with protective services, adoption/foster care issues, and reporting requirements
- Familiarity with major disease processes such as cancer, CHF, COPD, dementia, diabetes, and stroke
- Strong communication, documentation, and patient/caregiver education abilities
- Ability to work consultatively across Emergency, Children’s, and Adult inpatient teams
