Roles & Responsibilities

• Conduct thorough psychosocial and biopsychosocial assessments to identify patient and family needs
• Develop and implement person-centered discharge plans in collaboration with the interdisciplinary team
• Provide brief clinical interventions, suicide screening, and safety planning as indicated
• Coordinate community resources and referrals to support safe transitions of care
• Document assessments, plans, interventions, and responses clearly in the medical record (Epic)
• Work closely with RN case managers and other disciplines to facilitate continuity of care
• Complete required reports of harm per state statutes and follow mandated reporting procedures
• Float within scope to meet facility needs across Emergency, Children’s, and Adult teams

Qualifications

• MSW from a CSWE-accredited program preferred; LCSW or LMSW accepted
• AK license as LMSW or LCSW required (pending AK license accepted)
• Minimum 1 year of professional experience required; 2 years preferred, ideally with acute care experience
• Comfortable with inpatient acute care settings including ED, ICU, medical-surgical and pediatrics
• Experience with Epic charting preferred
• Strong assessment, crisis intervention, safety planning, and discharge planning skills
• Ability to collaborate effectively with interdisciplinary teams and maintain professional documentation

Education

• Master of Social Work (MSW) from a CSWE-accredited school
• LMSW or LCSW licensure (Alaska) — pending license acceptable during onboarding

Knowledge & Skills

• Case management, discharge planning, and care coordination expertise
• Biopsychosocial assessment, MMSE and geriatric assessment familiarity
• Suicide screening, safety planning, and high-risk intervention skills
• Knowledge of community resources: Medicaid, Medicare, transportation, housing, and social supports
• Experience with protective services, adoption/foster care issues, and reporting requirements
• Familiarity with major disease processes such as cancer, CHF, COPD, dementia, diabetes, and stroke
• Strong communication, documentation, and patient/caregiver education abilities
• Ability to work consultatively across Emergency, Children’s, and Adult inpatient teams
