Shift Type: Monday-Friday, 08:30 AM - 05:00 PM, Day shift (5x8 with required weekend coverage as needed)
Hours per Week: 40
Candidate Pay Rate: $57 per hour

Roles & Responsibilities
- Conduct thorough biopsychosocial assessments to understand patients and families and shape care plans
- Develop and coordinate safe, person-centered discharge plans in collaboration with the care team
- Provide short-term clinical interventions, crisis support, and safety planning including suicide risk screening
- Document care clearly and comprehensively in Epic to ensure continuity and quality
- Coordinate resources and referrals to community programs to support transitions of care
- Complete required state reports of harm and engage with protective services when appropriate
- Collaborate closely with RN case managers and interdisciplinary teams to achieve timely discharges
- Float within scope to meet facility needs across Emergency, Children’s, and Adult teams
Qualifications
- MSW from a CSWE-accredited program (Master’s in Social Work)
- Active Alaska LMSW or LCSW required (pending AK license accepted)
- Minimum: 1 year of experience (acute care experience required); 2 years preferred
- Comfort working across inpatient settings (Emergency Department, NICU/PICU, med-surg, ICU, maternity)
- Experience with discharge planning, case management, crisis intervention, and safety planning
- First-time travelers welcome; note: candidate permanent address must be at least 50 miles from the facility
Education
- Master of Social Work (MSW) from a CSWE-accredited program
- LMSW or LCSW licensure (Alaska) — pending AK license accepted
Knowledge & Skills
- Strong case management and care coordination skills
- Biopsychosocial assessment and short-term clinical intervention experience
- Suicide risk screening and safety planning expertise
- Knowledge of community resources (Medicaid, Medicare, housing, transportation, etc.)
- Familiarity with geriatric tools (MMSE, GDS) and assessment techniques
- Comfort documenting in Epic and producing accurate reports per state statutes
- Ability to collaborate with interdisciplinary teams and communicate clearly with patients/families
- Understanding of major disease processes (CHF, COPD, diabetes, stroke, cancer, psychiatric disorders)
