Roles & Responsibilities
- Evaluate patients and develop individualized occupational therapy treatment plans to improve independence
- Deliver evidence-based interventions that support activities of daily living (ADLs)
- Complete evaluations, progress notes, and discharge documentation in line with facility and regulatory guidelines
- Collaborate with interdisciplinary team members to coordinate comprehensive patient care
- Educate patients and families on therapy goals, adaptive techniques, and safety strategies to promote long-term success
- Maintain professional standards and ensure compliance with state, federal, and facility policies
Qualifications
- Active California Occupational Therapist license in good standing
- Graduate of an accredited Occupational Therapy program
- Skilled Nursing Facility or rehabilitation experience preferred
- Strong communication, clinical reasoning, and documentation skills
- Ability to work independently and as part of a multidisciplinary team
Education
- Graduate of an accredited Occupational Therapy program
Knowledge & Skills
- Occupational Therapy clinical knowledge and patient evaluation expertise
- Treatment planning for ADLs and functional recovery
- Clear, compliant documentation and progress reporting
- Effective communication and patient/family education
- Clinical reasoning and interdisciplinary collaboration
- Familiarity with skilled nursing facility and rehabilitation workflows
