Roles & Responsibilities
- Evaluate patients and design individualized physical therapy treatment plans that make a real difference
- Deliver evidence-based interventions to improve mobility, strength, balance, and independence
- Complete thorough evaluations, progress notes, and documentation in line with facility and regulatory standards
- Collaborate with the interdisciplinary team to coordinate comprehensive patient care
- Educate patients and families on goals, safety precautions, and home exercise plans to support lasting progress
- Maintain compliance with state, federal, and facility policies to ensure safe, high-quality care
Qualifications
- Active California Physical Therapist license in good standing (required)
- Graduate of an accredited Physical Therapy program
- Experience in skilled nursing facility or rehabilitation settings preferred
- Strong clinical judgment with excellent documentation and communication skills
- Comfortable working independently and adapting to varying patient volumes
Education
- Graduate of an accredited Physical Therapy program
Knowledge & Skills
- Skilled in patient evaluation and individualized treatment planning
- Proficient with evidence-based therapeutic interventions for mobility, strength, and balance
- Excellent clinical documentation practices and familiarity with regulatory requirements
- Strong interdisciplinary collaboration and communication
- Ability to prioritize caseloads and adapt to changing clinical needs
