Roles & Responsibilities
- Provide compassionate, high-quality direct patient care as an LPN during 12-hour night shifts
- Administer medications and treatments accurately and document all care in the chart
- Monitor and report changes in patient condition to the RN and interdisciplinary team
- Assist residents with activities of daily living while promoting dignity and comfort
- Support admission, discharge and transfer processes to ensure smooth transitions
- Participate in care planning and follow facility policies and infection control standards
- Communicate clearly with families and team members to deliver person-centered care
Qualifications
- Current LPN license in West Virginia (or ability to obtain prior to start)
- Basic Life Support (BLS/CPR) certification preferred or obtained upon hire
- Prior experience in long-term care or skilled nursing is preferred
- Comfortable working 12-hour night shifts and participating in rotating weekend coverage
- Strong communication, time management and team collaboration skills
Education
- Graduate of an accredited Practical Nursing (LPN) program or equivalent
Knowledge & Skills
- Solid knowledge of medication administration and nursing fundamentals
- Comfortable with electronic charting and accurate documentation
- Skilled at monitoring vital signs, wound care basics and reporting clinical changes
- Able to prioritize tasks in a fast-paced night shift environment
- Professional bedside manner and ability to work within an interdisciplinary team
