Roles & Responsibilities

• Implement occupational therapy treatment plans under the direction of the supervising OT to help patients regain independence
• Deliver meaningful, client-centered interventions focused on activities of daily living, adaptive equipment training, and therapeutic activities
• Document patient progress and outcomes accurately and promptly in the facility EMR to support continuity of care
• Collaborate with nurses, therapists, caregivers and family members to create coordinated care plans
• Educate patients and families on home programs, safety strategies and adaptive techniques to maximize carryover
• Maintain a safe, professional environment and adhere to infection control, safety and documentation standards

Qualifications

• Active COTA credential (Certified Occupational Therapy Assistant) required
• Current California OTA licensure/registration or ability to obtain state registration prior to start
• Basic Life Support (BLS/CPR) certification required or obtain upon hire
• Comfortable providing care in skilled nursing/rehab settings and working collaboratively with a multidisciplinary team
• Ability to pass background check and drug screen and meet facility onboarding requirements
• Reliable transportation and flexibility to work the assigned onsite schedule

Education

• Associate degree from an accredited Occupational Therapy Assistant (OTA) program
• Graduation from an ACOTE-accredited OTA program preferred

Knowledge & Skills

• Skilled in delivering ADL-focused interventions and using adaptive equipment
• Strong documentation and clinical charting skills in a timely manner
• Excellent communication and interpersonal skills for patient and team interactions
• Ability to follow treatment plans, modify activities based on patient response, and report changes to supervising OT
• Familiarity with common rehab modalities, positioning, and safety techniques in post-acute care
