Roles & Responsibilities

• Assess patient function and needs using evidence-based evaluation techniques
• Develop individualized treatment plans that promote mobility, independence and safety
• Deliver skilled therapeutic interventions including therapeutic exercise and gait training
• Document progress clearly and timely in the patient record and coordinate care plans
• Collaborate with interdisciplinary team members to optimize patient outcomes
• Educate patients and families to support home exercise programs and discharge success
• Ensure a safe therapeutic environment and advocate for patient-centered care

Qualifications

• Active Physical Therapy license in California (required)
• Doctor of Physical Therapy (DPT) or equivalent clinical degree preferred
• Current BLS/CPR certification (required or must obtain upon hire)
• Experience in inpatient rehab, skilled nursing or outpatient therapy is a strong plus
• Excellent communication skills and a collaborative, patient-focused mindset
• Ability to manage caseload, prioritize tasks, and document accurately

Education

• Doctor of Physical Therapy (DPT) or equivalent degree in Physical Therapy

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and functional mobility training
• Experience with gait training, transfers, balance retraining and assistive devices
• Familiarity with common rehab modalities and manual therapy techniques
• Proficient documentation skills and comfort with electronic medical records
• Strong clinical reasoning, safety awareness, and patient/family education techniques
