Roles & Responsibilities
- Conduct thorough initial and ongoing patient assessments in the home setting to guide care
- Develop, update and implement measurable home health plans of care in collaboration with physicians and the interdisciplinary team
- Deliver skilled physical therapy interventions to relieve pain, restore function and maximize patient independence
- Provide patient and caregiver education on treatment, disease management, equipment use and safety
- Document clinical care, time and travel accurately and in a timely manner in accordance with regulations
- Coordinate with physicians, nurses and other care providers to ensure seamless care and prevent hospital readmissions
- Supervise home health aides and physical therapy assistants and document supervision appropriately
- Perform OASIS data collection and submit assessments according to CMS guidance and agency policy
Qualifications
- Graduate of a CAPTE-accredited Physical Therapist education program or equivalent evaluation for internationally educated applicants
- Active Physical Therapist license in Iowa (compact allowed)
- Passed the state-approved examination for Physical Therapists
- Minimum of 1 year experience caring for a similar patient population
- Basic knowledge of commonly used medications, their actions and side effects
- Ability to perform assessments and treatments independently in the home setting
- Reliable transportation and ability to drive safely in all types of weather
- Proficiency with basic Microsoft Office functions and electronic documentation
Education
- Graduate of a Physical Therapist education program approved by CAPTE or an equivalent program as determined by an approved credentials evaluation organization
Knowledge & Skills
- Patient assessment and goal-driven plan of care development
- Skilled rehabilitation treatments and use of modalities appropriate to home health
- Clinical documentation best practices and OASIS data collection
- Care coordination and interdisciplinary communication
- Patient and caregiver education techniques for improved outcomes
- Supervision of home health aides and physical therapy assistants
- Working knowledge of prosthetic and orthotic devices
- Understanding of infection control and safe use of equipment
