Roles & Responsibilities

• Provide compassionate, direct patient care and monitor patient condition throughout the shift
• Administer medications and treatments as ordered, following facility policies
• Collaborate with the interdisciplinary team to implement individualized care plans
• Document patient progress, interventions, and responses accurately and timely
• Support patients and families with education, comfort, and clear communication
• Assist with admissions, discharges, and transfers to ensure smooth transitions
• Maintain a safe environment by following infection control and safety protocols

Qualifications

• Current LPN license in Delaware or ability to obtain state licensure prior to start
• Previous experience in a long-term care or acute care setting preferred
• Current Basic Life Support (BLS) certification preferred
• Strong clinical assessment and medication administration skills
• Excellent communication, teamwork, and time management abilities
• Ability to pass required background and health screenings per facility policy

Education

• LPN: Vocational nursing diploma or LPN certificate required
• RN option (if applicable): Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Solid knowledge of nursing fundamentals and medication administration
• Skilled in charting, clinical documentation, and electronic health records
• Comfortable working within care teams and following care plans
• Ability to prioritize tasks in a fast-paced environment
• Patient-centered approach with strong observational skills
