Roles & Responsibilities

Provide compassionate, patient-centered nursing care and act as an advocate for residents
Assess patient condition, monitor vital signs and document clinical observations accurately
Administer medications and treatments safely and in accordance with policies
Collaborate with interdisciplinary teams to develop and update individualized care plans
Educate patients and families about care plans, medications and discharge instructions
Respond promptly to changes in patient condition and escalate concerns to providers

Qualifications

Active Registered Nurse license (RN) — ability to practice in Connecticut required
Current BLS certification; ACLS or other specialty certifications preferred when applicable
Demonstrated clinical experience in long term care or skilled nursing settings is a plus
Strong communication skills and ability to work well in a collaborative team
Reliable documentation and time management skills

Education

Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

Clinical assessment and patient monitoring
Medication administration and safety protocols
Care planning and interdisciplinary collaboration
Electronic medical record documentation
Infection control and patient safety practices
Effective communication with patients, families and clinical teams
