Roles & Responsibilities

Provide compassionate direct patient care and assist with activities of daily living
Take and document vital signs accurately and report changes promptly
Assist nursing staff with resident mobility, hygiene, and safe transfers
Maintain a clean, safe environment and follow infection control practices
Communicate clearly with residents, families, and the care team to support great outcomes
Document care activities thoroughly and timely in resident charts
Collaborate with the interdisciplinary team to support individualized care plans

Qualifications

Current CNA certification (active in Pennsylvania) required
Comfortable working 8-hour shifts; reliable attendance is important
Strong communication and teamwork skills; resident-focused attitude
Basic ability to lift/assist residents as required and follow safe handling practices
Willingness to complete facility onboarding requirements (background check, immunizations, TB screening)

Education

CNA certification (required)
High school diploma or GED preferred

Knowledge & Skills

Knowledge of patient hygiene, ADLs, and safe transfer techniques
Skill in taking and recording vital signs accurately
Infection prevention and basic clinical safety practices
Good observational skills to notice and report changes in condition
Teamwork, time management, and respectful resident-centered communication
