Roles & Responsibilities

Provide compassionate direct patient care within the scope of LPN practice
Administer medications and document treatments accurately and timely
Monitor patient status and report changes to the interdisciplinary team
Maintain clear, professional documentation in the patient record
Support patient comfort and safety while promoting dignity and respect
Collaborate with nurses and care team members to deliver seamless care

Qualifications

Valid Licensed Practical Nurse (LPN) credential (Licensed Practical Nurse / LPN)
Active license to practice in Pennsylvania (or eligibility to obtain one)
Demonstrated ability to provide safe, patient-centered care
Strong communication skills and ability to work well in a team

Knowledge & Skills

Clinical nursing skills appropriate to the LPN scope of practice
Medication administration and documentation
Basic patient assessment and monitoring
Effective communication with patients, families, and multidisciplinary teams
Time management and organization in a fast-paced clinical setting
