Shift Type: Sunday-Saturday, 7:00 PM - 7:30 AM NOC (Night shift) — every other weekend off rotation

Roles & Responsibilities
- Provide high-quality, compassionate bedside care to residents as a licensed practical nurse
- Administer medications and treatments safely and accurately according to orders
- Perform and document thorough patient assessments and report changes to RN/MD
- Collaborate with the interdisciplinary team to develop and implement care plans
- Maintain accurate, timely charting in the electronic medical record
- Support admissions, discharges and transfers with professionalism and care
- Champion infection control and patient safety practices on every shift
Qualifications
- Current, unencumbered LPN license appropriate for practice in West Virginia
- Basic Life Support (BLS) certification required; additional certifications a plus
- Experience in long-term care or skilled nursing preferred but new grads encouraged to apply
- Ability to pass background check, drug screen and meet facility onboarding requirements
- Strong communication skills and a team-oriented attitude
Education
- Graduate of an accredited Practical Nursing (LPN/Vocational Nursing) program
Knowledge & Skills
- Solid medication administration and documentation skills
- Comfortable with resident assessment, wound care, and ADL support
- Familiarity with electronic medical records and charting best practices
- Good clinical judgment, time management and prioritization under busy conditions
- Excellent interpersonal skills for working with residents, families and staff
