Roles & Responsibilities

• Evaluate patients' functional status and develop individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve mobility and independence
• Document progress thoroughly and communicate outcomes with the interdisciplinary team
• Collaborate with nurses, physicians, and case managers to coordinate patient care
• Educate patients and families on home programs, safety, and strategies to maintain gains
• Monitor and modify treatment plans based on patient response and clinical goals
• Contribute to a positive, patient-focused culture that prioritizes safe recovery

Qualifications

• Current Physical Therapist license in New Mexico (or eligible to obtain)
• Graduation from an accredited Physical Therapy program (DPT preferred)
• Experience in rehabilitation settings preferred (inpatient, outpatient, or skilled nursing)
• Strong clinical assessment and therapeutic exercise skills
• Excellent communication skills and ability to work within an interdisciplinary team
• Agencies: must have a New Mexico caregivers account as noted in facility requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy or related degree acceptable when state-licensed

Knowledge & Skills

• Skilled in patient evaluation, therapeutic exercise, and manual therapy techniques
• Proficient with documenting clinical notes and care plans in the medical record
• Knowledge of mobility aids, adaptive equipment, and fall-prevention strategies
• Ability to set measurable goals and track patient outcomes
• Comfortable teaching patients and families practical home programs
• Professionalism, time management, and strong interpersonal skills
