Roles & Responsibilities
- Evaluate patients' functional status and develop individualized treatment plans
- Deliver evidence-based therapeutic interventions to improve mobility and independence
- Document progress thoroughly and communicate outcomes with the interdisciplinary team
- Collaborate with nurses, physicians, and case managers to coordinate patient care
- Educate patients and families on home programs, safety, and strategies to maintain gains
- Monitor and modify treatment plans based on patient response and clinical goals
- Contribute to a positive, patient-focused culture that prioritizes safe recovery
Qualifications
- Current Physical Therapist license in New Mexico (or eligible to obtain)
- Graduation from an accredited Physical Therapy program (DPT preferred)
- Experience in rehabilitation settings preferred (inpatient, outpatient, or skilled nursing)
- Strong clinical assessment and therapeutic exercise skills
- Excellent communication skills and ability to work within an interdisciplinary team
- Agencies: must have a New Mexico caregivers account as noted in facility requirements
Education
- Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy or related degree acceptable when state-licensed
Knowledge & Skills
- Skilled in patient evaluation, therapeutic exercise, and manual therapy techniques
- Proficient with documenting clinical notes and care plans in the medical record
- Knowledge of mobility aids, adaptive equipment, and fall-prevention strategies
- Ability to set measurable goals and track patient outcomes
- Comfortable teaching patients and families practical home programs
- Professionalism, time management, and strong interpersonal skills
