Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on and therapeutic interventions to restore mobility and function
• Measure and document patient progress clearly and communicate outcomes to the care team
• Collaborate with physicians, nurses and rehab staff to optimize patient care and discharge planning
• Educate patients and families on home programs, safety and strategies to maintain gains
• Participate in team meetings and contribute to a positive rehabilitation culture

Qualifications

• Current Physical Therapist license in New Hampshire or ability to obtain prior to start
• Minimum experience in inpatient rehabilitation or skilled nursing preferred
• Current BLS/CPR certification
• Comfortable with thorough documentation and electronic medical records
• Strong communication skills and a patient-centered approach

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled with mobility training, therapeutic exercise and gait training
• Familiarity with ADL retraining and assistive device prescription
• Good clinical reasoning and outcome measurement (FIM, 6MWT, TUG, etc.)
• Ability to work effectively in a fast-paced inpatient rehab environment
• Strong interpersonal skills for teamwork and family education
