Roles & Responsibilities

• Evaluate and create individualized treatment plans that restore mobility and function
• Deliver hands-on therapy and evidence-based interventions to improve patient outcomes
• Collaborate with nursing and rehab teams to coordinate holistic care plans
• Document progress clearly and timely in the facility’s clinical records
• Educate patients and families on home programs and safety to support long-term recovery
• Participate in team meetings and discharge planning to ensure seamless transitions

Qualifications

• Active Physical Therapist license in Pennsylvania (or compact/eligible) required
• Degree in Physical Therapy (DPT preferred) from an accredited program
• Current CPR/BLS certification
• Experience in skilled nursing facility or outpatient rehab preferred
• Strong clinical assessment and manual therapy skills with excellent documentation
• Clean background check and ability to meet facility credentialing requirements

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy or related field accepted

Knowledge & Skills

• Skilled in musculoskeletal and neurological assessment
• Familiarity with therapeutic modalities, gait training, balance and fall prevention
• Proficient with clinical documentation and electronic medical records
• Strong communication and patient education abilities
• Ability to collaborate with interdisciplinary teams and lead care planning
• Time management and caseload prioritization for efficient, quality care
