Roles & Responsibilities

• Assess patients and design individualized physical therapy plans that drive real progress
• Deliver evidence-based interventions including therapeutic exercise and manual techniques
• Document evaluations, progress notes, and discharge summaries thoroughly and on time
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Educate patients and families on home exercise programs, safety, and long-term recovery
• Participate in discharge planning and ensure smooth transitions to next-level care
• Contribute to quality improvement initiatives and maintain compliance with facility policies

Qualifications

• Active California Physical Therapist license (required)
• Doctor of Physical Therapy (DPT) or equivalent degree preferred
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Prior experience in outpatient or rehab settings is preferred
• Strong clinical reasoning, documentation skills, and patient-centered communication
• Ability to adapt to a travel/contract role and integrate quickly into new teams

Education

• Doctor of Physical Therapy (DPT) or Master's in Physical Therapy; equivalent qualifications considered

Knowledge & Skills

• Skilled in therapeutic exercise prescription and manual therapy techniques
• Experience with gait training, balance interventions, and mobility equipment
• Proficient in clinical documentation and use of electronic medical records
• Solid understanding of discharge planning and interdisciplinary care coordination
• Strong time management, prioritization, and patient education skills
• Comfortable working with diverse patient populations and varying acuity levels
