Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents, prioritizing safety and dignity
• Perform focused patient assessments and report changes in condition promptly
• Administer medications and treatments accurately and document according to facility policy
• Assist with activities of daily living and promote resident independence whenever possible
• Collaborate with the interdisciplinary team to support individualized care plans
• Maintain clear, timely documentation in the resident record and communicate effectively with families and caregivers

Qualifications

• Active LPN license in West Virginia or ability to obtain state licensure prior to start
• Completion of an accredited Practical Nursing program (LPN/LVN)
• Current BLS/CPR certification required; other certifications welcomed
• Comfort and experience working 12-hour night shifts preferred
• Strong clinical judgment, attention to detail and ability to prioritize in a fast-paced setting
• Up-to-date immunizations and ability to meet facility onboarding requirements

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program

Knowledge & Skills

• Medication administration and documentation best practices
• Basic wound care and infection control procedures
• Ability to perform reliable vital signs assessment and focused clinical observations
• Familiarity with electronic medical records and clear charting
• Effective communication with residents, families and interdisciplinary teams
• Time management, delegation and teamwork skills appropriate for night shift dynamics
