Roles & Responsibilities
- Perform thorough patient evaluations to assess functional performance and rehabilitation needs
- Develop and implement personalized treatment plans that focus on independence in daily living
- Deliver therapeutic activities, exercises, and adaptive strategies to maximize patient outcomes
- Monitor and document patient progress while ensuring compliance with state and federal guidelines
- Educate patients and families on therapy goals, adaptive equipment, and home programs
- Collaborate actively with an interdisciplinary team (PTs, nurses, physicians, SLPs) to coordinate care
Qualifications
- Active and current Occupational Therapist (OT) license in California (required)
- Graduate of an accredited Occupational Therapy program (OTD or equivalent)
- Previous experience working in a skilled nursing facility (SNF) required
- Strong clinical judgment with a patient-centered approach
- Excellent communication, organization, and teamwork skills
Education
- Doctorate in Occupational Therapy (OTD) or equivalent from an accredited program
Knowledge & Skills
- Skilled in patient evaluation, treatment planning, and progress documentation
- Proficient with therapeutic activities and adaptive strategies to support independence
- Solid knowledge of regulations and best practices for SNF care and documentation
- Strong interpersonal skills for patient education and interdisciplinary collaboration
- Organized and detail-oriented with excellent communication abilities
