Roles & Responsibilities
- Conduct comprehensive initial and annual foot assessments, including diabetes history and medication review
- Examine foot structure, circulation, skin, and nails and document findings thoroughly
- Provide routine foot care services such as nail trimming/filing, reduction of thick nails, callus and corn care, and management of ingrown nail portions
- Evaluate footwear for safety and fit and make recommendations to reduce risk
- Coordinate referrals to medical providers when further evaluation or treatment is needed
- Educate patients on foot care, prevention strategies, and available resources to reduce complications
- Maintain accurate and timely documentation in the electronic health record to support continuity of care
- Promote preventive care and early detection to improve patient outcomes
Qualifications
- Current unencumbered Oregon RN license required upon submission
- Healthcare Provider BLS certification required
- Certification as a Diabetes Educator (CDE/CDCES) preferred
- Bachelor's degree in Nursing preferred
- Minimum of three (3) years clinical experience as an RN or RD, with emphasis on diabetes care and/or chronic disease management preferred
- Prior teaching experience preferred
Education
- Bachelor's degree in Nursing preferred
Knowledge & Skills
- Expertise in diabetes-related foot assessments and routine foot care techniques
- Strong patient education and coaching skills focused on prevention
- Comfortable with EHR documentation and maintaining accurate clinical records
- Ability to evaluate footwear and recommend safe options
- Referral coordination and interdisciplinary communication skills
- Infection control, skin and nail assessment, and wound-awareness
- Compassionate, patient-centered approach and strong clinical judgment
