Contract Type: Travel Contract
Shift Type: Saturday-Sunday (every weekend commitment), 10:00 PM - 6:30 AM NOC (night shift). Additional weekday night shifts may be scheduled as needed.
Hours per Week: 40
Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide compassionate, safe bedside care to residents during night shifts, ensuring comfort and dignity
- Administer medications accurately and monitor patient responses, reporting changes to the care team
- Perform thorough patient assessments and document findings clearly in the chart/EHR
- Collaborate with RNs, providers, and interdisciplinary team members to coordinate care plans
- Support activities of daily living and promote mobility and independence where appropriate
- Respond promptly to urgent situations and escalate concerns per facility protocol
- Maintain infection control standards and follow facility policies for safety and documentation
- Honor the facility's weekend commitment requirement and communicate schedule needs in advance
Qualifications
- Current Licensed Practical Nurse (LPN) license in Vermont or eligibility to obtain Vermont licensure
- Minimum clinical experience providing direct patient care in long-term care, skilled nursing, or similar settings preferred
- Current BLS/CPR certification required
- Reliable transportation (must have own transportation for this assignment)
- Ability to work nights and fulfill a weekend commitment as specified by the facility
- Strong communication skills and ability to work as part of a multidisciplinary team
Education
- Graduate of an accredited Practical Nursing (LPN) program
Knowledge & Skills
- Solid knowledge of medication administration and documentation practices
- Skilled in patient assessment, wound care basics, and recognizing changes in condition
- Comfortable using electronic health records and accurate charting
- Good time management and prioritization during busy night shifts
- Familiarity with infection control and safety protocols in a long-term care environment
- Team player attitude with a focus on compassionate resident-centered care
