Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents using evidence-based practices
• Administer medications and treatments accurately and document thoroughly in the medical record
• Monitor patient status, assess changes, and escalate concerns to the care team promptly
• Collaborate with nurses, therapists, and support staff to develop and implement individualized care plans
• Educate residents and families about care plans, medications, and health maintenance
• Maintain a safe, clean environment and follow infection control and facility policies

Qualifications

• Current LPN license (Licensed Practical Nurse) — required
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification required or willingness to obtain
• Comfortable working 12-hour night shifts and flexible to the posted schedule
• Strong communication skills and the ability to work as part of a multidisciplinary team
• Previous experience in long-term care or skilled nursing preferred but not mandatory

Education

• Graduate of an accredited practical nursing (LPN) program; state LPN licensure required

Knowledge & Skills

• Skilled in medication administration, wound care basics, and routine clinical assessments
• Proficient with documentation and electronic health records (EHR)
• Knowledge of infection control practices and resident safety protocols
• Excellent interpersonal skills and the ability to build rapport with residents and families
• Time management and prioritization skills for fast-paced shift environments
