Roles & Responsibilities

• Evaluate residents and establish individualized treatment plans with clear short- and long-term functional goals
• Provide ADL training and hands-on interventions to maximize residents' independence
• Assist with transfers, ambulation, positioning and safe use of mechanical lifts
• Recommend, obtain, and train residents/caregivers on appropriate assistive devices
• Document all evaluative and therapeutic interactions accurately in the EMR
• Collaborate with physicians and the interdisciplinary care team on resident care and discharge planning
• Monitor treatment progress, modify plans of care as needed, and report outcomes
• Supervise and mentor assistant-level staff and support new staff orientation
• Maintain a clean, safe clinical environment and follow infection control procedures

Qualifications

• Licensed Occupational Therapist in the State of Pennsylvania (required)
• Bachelor's degree in Occupational Therapy (required); Master's or Doctorate preferred
• Basic Life Support (BLS) certification preferred
• Prior experience in long-term care or skilled nursing facility preferred
• Experience with EMR documentation and Microsoft Office
• Knowledge of Medicare/Medicaid reimbursement rules and clinical documentation standards
• Strong verbal and written communication skills and ability to work in interdisciplinary teams

Education

• Bachelor's degree in Occupational Therapy (required)
• Master's degree or Doctorate in Occupational Therapy (preferred)

• Occupational therapy evaluation and treatment planning

• Transfers, mobility training, and mechanical lift use
• Accurate EMR documentation and charting
• Understanding of Medicare/Medicaid reimbursement regulations
