Roles & Responsibilities

• Evaluate residents and develop individualized treatment plans to restore mobility and independence
• Deliver hands-on therapy, therapeutic exercise, and functional training focused on safety and quality of life
• Collaborate with nurses, aides, and interdisciplinary team members to coordinate care and optimize outcomes
• Document treatments and progress accurately and timely in resident records
• Educate residents and families on home programs, adaptive strategies, and fall prevention
• Monitor clinical outcomes and adjust plans of care to ensure measurable progress

Qualifications

• Licensed Physical Therapist (PT) or Doctor of Physical Therapy (DPT) — Illinois licensure or ability to obtain required state license
• Open to all experience levels, including new grads
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Comfort working in a skilled nursing facility setting and collaborating with an interdisciplinary team
• Strong documentation skills and ability to manage a caseload efficiently

Education

• DPT preferred;/licensed PT (per state requirements)

Knowledge & Skills

• Skilled in geriatric rehabilitation and SNF care principles
• Ability to perform transfers, gait training, balance and endurance programs
• Proficient with documentation standards and skilled therapy justification
• Excellent communication and patient/family education skills
• Problem-solving mindset to adapt plans for complex medical conditions
