Roles & Responsibilities

• Evaluate patients’ functional mobility and develop individualized treatment plans that help them regain independence
• Deliver skilled therapeutic interventions including therapeutic exercise, gait training, balance and transfer training
• Collaborate with nurses, occupational therapists, speech therapists and physicians to coordinate patient-centered care
• Document patient progress clearly and timely in the electronic medical record to support outcomes and reimbursements
• Educate patients and families on therapy goals, home exercises and safe mobility strategies
• Participate in care conferences and discharge planning to ensure smooth transitions and optimal outcomes

Qualifications

• Licensed Physical Therapist in Virginia or eligible for Virginia licensure
• Current CPR/BLS certification (or willing to obtain)
• Experience in skilled nursing facility (SNF) or post-acute care preferred
• Strong clinical assessment and manual therapy skills with excellent communication
• Ability to work collaboratively in a team-focused environment and support weekend rotation

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in related field acceptable with required licensure

Knowledge & Skills

• Proven ability in gait training, transfer techniques and therapeutic exercise prescription
• Skilled in mobility aids, adaptive equipment recommendations and fall prevention
• Comfortable with documentation in EMR systems and meeting regulatory documentation standards
• Good clinical judgment, time management and patient/family education skills
• Familiarity with Medicare and post-acute care reimbursement and documentation expectations
