Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document per policy
• Monitor and record vital signs, reporting changes in condition promptly
• Collaborate with the interdisciplinary care team to implement individualized care plans
• Support residents with activities of daily living while promoting dignity and independence
• Maintain accurate, timely documentation in the resident chart and electronic record
• Follow infection control practices and facility safety protocols to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license (eligible to work in Pennsylvania)
• Current Basic Life Support (BLS) certification or ability to obtain prior to start
• Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
• Strong communication skills and ability to work collaboratively in a care team
• Ability to pass facility-required background checks and health screenings

Education

• Completion of an accredited LPN/LVN program
• Current LPN/LVN state license

Knowledge & Skills

• Medication administration and documentation best practices
• Assessment of common geriatric conditions and changes in patient status
• Basic wound care and skilled nursing support
• Accurate vital sign measurement and trending
• Comfort with electronic charting and timely documentation
• Infection control, safety procedures, and resident-centered care principles
