Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and treatments safely and accurately following facility protocols
• Perform skilled assessments and document changes in patient condition clearly and promptly
• Collaborate with the interdisciplinary team to create and follow individualized care plans
• Support residents with daily living activities while promoting independence and dignity
• Respond to urgent situations calmly and efficiently, escalating concerns when needed
• Communicate effectively with families and team members to ensure continuity of care

Qualifications

• Current Pennsylvania LPN license in good standing
• Current BLS/CPR certification (or willingness to obtain prior to start)
• Up-to-date immunizations and ability to pass required health screenings
• Successful background check and drug screen as required by the facility
• Comfortable working overnight shifts and providing direct resident care
• Strong communication skills and a team-oriented attitude

Education

• LPN diploma or vocational nursing certificate; active LPN license required

Knowledge & Skills

• Medication administration and accurate charting
• Basic wound care and routine clinical procedures
• Resident assessment, vital signs, and recognizing changes in condition
• Electronic medical record (EMR) documentation
• Time management, prioritization, and teamwork in a fast-paced setting
• Empathy, patient advocacy, and clear communication with families
