Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with LPN scope
• Administer medications and document administration accurately and promptly
• Perform routine assessments and report changes in condition to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to optimize patient outcomes
• Maintain accurate, timely patient records in the facility charting system
• Support residents and families with clear communication and respectful care

Qualifications

• Active Licensed Practical Nurse (LPN) license in Pennsylvania required
• Current CPR/BLS certification required (or obtain upon hire)
• Previous LPN experience in long-term care or skilled nursing preferred
• Valid TB test and up-to-date immunizations per facility policy
• Ability to pass background check and drug screening as required

Education

• Graduation from an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Strong clinical assessment and medication administration skills
• Comfortable with wound care, IV care according to facility scope, and ADL support
• Proficient with electronic medical records and accurate charting
• Excellent communication and teamwork skills
• Reliable time management and ability to prioritize in a busy care setting
