Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents following care plans and facility protocols
• Administer medications and treatments accurately and document patient responses
• Assess resident condition and communicate changes promptly to the care team
• Collaborate with RNs, therapists, and support staff to deliver coordinated care
• Support residents with activities of daily living while promoting dignity and independence
• Maintain accurate, timely documentation in clinical records and shift reports
• Participate in interdisciplinary meetings and contribute to quality improvement efforts

Qualifications

• Valid LPN license in the state of practice (active and unencumbered)
• Current basic life support (BLS/CPR) certification
• Prior nursing experience in long-term care or skilled nursing preferred
• Strong communication skills and a collaborative attitude
• Ability to pass facility onboarding, health screening, and background checks

Education

• LPN diploma or equivalent from an accredited program
• Additional continuing education in geriatric or long-term care a plus

Knowledge & Skills

• Solid clinical assessment and medication administration skills
• Familiarity with long-term care documentation and electronic charting
• Ability to handle multiple priorities in a fast-paced environment
• Team-oriented approach and strong resident-focused bedside manner
• Basic knowledge of infection control and safety procedures
