Roles & Responsibilities

• Provide compassionate direct patient care aligned with facility policies and care plans
• Administer medications and treatments accurately and document in the patient record
• Assess patients regularly and report changes in condition to the clinical team
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Maintain accurate, timely clinical documentation and support quality initiatives
• Educate patients and families about care plans, medications, and discharge instructions

Qualifications

• Current Licensed Practical Nurse (LPN) license in Pennsylvania
• Basic Life Support (BLS/CPR) certification or willingness to obtain promptly
• Clinical experience in long-term care, skilled nursing, or similar settings preferred
• Ability to pass background check and drug screen as required for travel assignments
• Strong communication skills, professionalism, and a patient-centered attitude

Education

• Graduation from an accredited practical nursing program (diploma or certificate)

Knowledge & Skills

• Solid patient assessment and clinical observation skills
• Medication administration and documentation best practices
• Familiarity with infection control and safety standards
• Comfort using electronic medical records (EMR) systems
• Effective time management, prioritization, and teamwork
