Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and safely
• Monitor vital signs and report changes in condition promptly
• Document care, observations and interventions thoroughly
• Collaborate with nurses, therapists and interdisciplinary team members
• Support patient comfort, dignity and infection prevention practices
• Educate patients and families about care plans and follow-up needs

Qualifications

• Active LPN license in good standing (required)
• Current CPR/BLS certification (required or obtain upon hire)
• Previous LPN experience preferred but new grads with strong clinical skills considered
• Ability to pass background check and required health screenings
• Strong communication skills and a team-oriented attitude

Education

• Practical Nursing diploma or equivalent

Knowledge & Skills

• Medication administration and documentation best practices
• Basic clinical assessment and vital signs monitoring
• Wound care, ADL support and resident-centered care approaches
• Electronic medical record navigation and accurate charting
• Effective communication with patients, families and staff
• Time management and prioritization in a fast-paced setting
