Roles & Responsibilities

• Perform thorough speech, language, cognitive-communication, and swallowing evaluations to create meaningful treatment plans
• Deliver individualized therapy that improves communication, safety, and quality of life for each patient
• Collaborate with nursing, therapy, and interdisciplinary teams to coordinate patient-centered care
• Document progress accurately and timely in the facility EHR and maintain compliance with regulatory standards
• Provide discharge planning recommendations and caregiver education to support long-term outcomes
• Float to nearby facilities as requested while maintaining consistent quality of care

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program
• Active Kentucky SLP license required prior to start
• ASHA Certificate of Clinical Competence (CCC-SLP) preferred
• Current Basic Life Support (BLS/CPR) certification preferred
• Experience in skilled nursing, rehab, or acute care settings is strongly preferred
• Strong documentation skills and comfortable working with electronic health records
• Professional, team-oriented approach with excellent communication skills

Education

• Master's degree in Speech-Language Pathology (required)
• ASHA CCC-SLP (preferred)

Knowledge & Skills

• Skilled in assessment and treatment of dysphagia, aphasia, apraxia, and cognitive-communication disorders
• Familiarity with evidence-based therapy techniques and measurable goal writing
• Ability to provide caregiver training and patient education
• Competence in interdisciplinary collaboration and discharge planning
• Good clinical documentation and time management skills
• Adaptability to float between nearby facilities when needed
