Roles & Responsibilities

• Assess, plan, and provide skilled physical therapy interventions to improve patient mobility and function
• Develop individualized treatment plans and document progress clearly and timely
• Collaborate with interdisciplinary teams to coordinate patient-centered care
• Educate patients and families on home exercise programs and injury prevention
• Use clinical reasoning to modify treatments based on patient response
• Participate in discharge planning to ensure safe transitions and continuity of care

Qualifications

• Valid Physical Therapist license in North Carolina or ability to obtain state licensure prior to start
• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy preferred
• Current BLS/CPR certification
• Prior experience in skilled nursing or outpatient rehab preferred but enthusiastic new graduates may be considered
• Strong documentation and time-management skills for a fast-paced setting
• Excellent communication and teamwork skills; patient-focused attitude

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy accepted

Knowledge & Skills

• Skilled in therapeutic exercise, gait training, balance and transfer training
• Familiarity with documentation systems and insurance-focused charting
• Ability to assess functional mobility and develop measurable goals
• Comfort working with older adults and common musculoskeletal conditions
• Strong interpersonal skills to build rapport with patients and families
