Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a skilled nursing and rehab setting
- Administer medications and document administration accurately and on time
- Monitor and record vital signs, report changes in condition to the care team
- Support wound care, dressing changes, and basic clinical procedures within LPN scope
- Assist residents with activities of daily living and promote independence
- Collaborate with RNs, therapists and support staff to implement care plans
- Maintain clear, timely charting in the resident medical record and follow facility protocols
Qualifications
- Valid Licensed Practical Nurse (LPN) license in Massachusetts (required)
- Current BLS/CPR certification (preferred or obtain upon hire)
- Up-to-date immunizations and TB screening per facility policy
- Ability to pass background check and drug screening
- Strong communication skills and a team-first attitude
- Prior LPN experience in long-term care or rehabilitation is a plus
Education
- Practical Nursing diploma or certificate
- Active LPN licensure in Massachusetts
Knowledge & Skills
- Medication administration and accurate documentation
- Basic assessment skills and recognizing changes in patient status
- Familiarity with wound care and infection control practices
- Comfortable using electronic medical records and charting systems
- Good time management, prioritization and teamwork skills
- Ability to provide compassionate resident-centered care
