Roles & Responsibilities

• Evaluate and treat patients using evidence-based physical therapy interventions to improve mobility and function
• Develop individualized treatment plans and measurable goals that support safe discharge and quality outcomes
• Document evaluations, progress notes, and discharge summaries accurately and timely in the facility charting system
• Collaborate with nurses, occupational therapists, speech therapists, and physicians to coordinate exceptional care
• Educate patients and families on home exercise programs, safety strategies, and ongoing recovery steps
• Assist with transfers, gait training, balance retraining, and therapeutic exercise tailored to each patient
• Support unit goals and float to Skyline once a week as needed to ensure continuity of care

Qualifications

• Licensed Physical Therapist in Virginia or eligible for state licensure
• Current BLS/CPR certification or willingness to obtain prior to start
• Comfortable working in a skilled nursing/rehabilitation setting and adapting to different patient needs
• Strong clinical documentation skills and familiarity with common EMR systems
• Able to float once weekly and work collaboratively across interdisciplinary teams

Education

• Doctor of Physical Therapy (DPT) preferred or Bachelor's degree in Physical Therapy where applicable
• Graduation from an accredited physical therapy program and eligibility for state licensure

Knowledge & Skills

• Skilled in patient assessment, exercise progression, and functional mobility training
• Experience with transfers, gait training, balance and fall prevention strategies
• Familiarity with therapeutic modalities and adaptive equipment
• Strong communication, patient education, and interdisciplinary collaboration skills
• Efficient clinical documentation and time management in a fast-paced rehab environment
