Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as a Licensed Practical Nurse
• Administer medications and document accurately according to facility policy
• Monitor and record vital signs, reporting any changes to the care team
• Assist with patient hygiene, mobility, and activities of daily living to improve comfort and recovery
• Collaborate with RNs, therapists, and support staff to deliver coordinated care
• Maintain accurate, timely documentation in the electronic medical record
• Follow infection control and safety protocols to protect patients and staff
• Support patient and family education with clear, empathetic communication

Qualifications

• Current LPN license in Pennsylvania required
• Graduate of an accredited practical nursing program
• Basic Life Support (BLS) certification required or must obtain upon hire
• Previous experience in long-term care, skilled nursing, or rehabilitation is a plus
• Strong communication, teamwork, and documentation skills

Education

• Graduate of an accredited LPN / Practical Nursing program
• Current LPN license in Pennsylvania

Knowledge & Skills

• Medication administration and basic dosage calculation
• Patient assessment and recognition of clinical changes
• Charting and familiarity with electronic medical records
• Infection control best practices and safety standards
• Effective communication with patients, families, and interdisciplinary teams
• Time management and prioritization in a busy clinical environment
