Roles & Responsibilities

• Provide skilled nursing care to rehabilitation patients with compassion and clinical accuracy
• Administer medications and treatments according to physician orders and facility policy
• Perform thorough patient assessments and document changes in condition promptly
• Collaborate with therapists, physicians and the interdisciplinary team to support patient progress
• Educate patients and families on care plans, adaptive strategies and discharge goals
• Support safe mobility, transfers and rehabilitation activities to maximize functional outcomes
• Maintain accurate electronic health records and complete timely shift handoffs
• Adhere to infection control, safety protocols and facility standards of care

Qualifications

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure (state licensure required)
• Experience in nursing with emphasis or background in rehabilitation preferred
• Current Basic Life Support (BLS) certification preferred
• Strong clinical assessment, medication administration and documentation skills
• Professional, team-oriented attitude with excellent communication skills

Knowledge & Skills

• Fundamentals of rehabilitation nursing and restorative care
• Medication administration and safe documentation practices
• Patient assessment, care planning and progress evaluation
• Ability to work effectively within an interdisciplinary rehabilitation team
• Familiarity with electronic health record documentation
• Time management, prioritization and patient-centered communication
