Roles & Responsibilities
- Provide skilled nursing care to rehabilitation patients with compassion and clinical accuracy
- Administer medications and treatments according to physician orders and facility policy
- Perform thorough patient assessments and document changes in condition promptly
- Collaborate with therapists, physicians and the interdisciplinary team to support patient progress
- Educate patients and families on care plans, adaptive strategies and discharge goals
- Support safe mobility, transfers and rehabilitation activities to maximize functional outcomes
- Maintain accurate electronic health records and complete timely shift handoffs
- Adhere to infection control, safety protocols and facility standards of care
Qualifications
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) licensure (state licensure required)
- Experience in nursing with emphasis or background in rehabilitation preferred
- Current Basic Life Support (BLS) certification preferred
- Strong clinical assessment, medication administration and documentation skills
- Professional, team-oriented attitude with excellent communication skills
Knowledge & Skills
- Fundamentals of rehabilitation nursing and restorative care
- Medication administration and safe documentation practices
- Patient assessment, care planning and progress evaluation
- Ability to work effectively within an interdisciplinary rehabilitation team
- Familiarity with electronic health record documentation
- Time management, prioritization and patient-centered communication
