Roles & Responsibilities

• Evaluate patients and develop individualized outpatient PT treatment plans that get people moving and back to life
• Provide hands-on and therapeutic interventions for orthopedic, neurologic, vestibular/BPPV and geriatric populations
• Deliver concise, timely documentation and charting in Cerner to maintain continuity of care
• Coordinate care with PTAs, Rehab Techs and the rehab team to optimize patient outcomes
• Monitor patient progress, modify treatment plans as needed, and communicate with the PT manager/director
• Implement falls management strategies and safety plans for a predominantly geriatric caseload

Qualifications

• Active Physical Therapy license or eligible for Arizona licensure (license-to-submit accepted; employer willing to wait)
• Minimum 1 year outpatient physical therapy experience
• BLS and CPR certification required
• Experience with EMR charting, preferably Cerner
• Strong documentation and patient evaluation skills
• Comfort working with geriatric patients and diagnoses such as ortho, neuro, vestibular/BPPV and falls

Education

• Doctor of Physical Therapy (DPT) preferred or Master’s in Physical Therapy; equivalent clinical experience considered

Knowledge & Skills

• Outpatient therapy best practices and evidence-based interventions
• Vestibular/BPPV assessment and treatment techniques
• Orthopedic and neurologic rehab principles
• Falls risk assessment and prevention strategies
• Geriatric care considerations and communication skills
• Proficiency with Cerner EMR and clear documentation/charting
• Team coordination with PTAs and Rehab Techs
