Roles & Responsibilities

• Evaluate, plan, and deliver individualized outpatient physical therapy care to improve patient function and independence
• Develop and implement evidence-based treatment plans and measurable goals
• Document assessments and treatments accurately in Cerner to ensure continuity of care
• Collaborate with physicians, nurses, and allied health staff for coordinated patient-centered care
• Provide safe inpatient coverage when needed, supporting continuity across settings
• Educate patients and families on home programs, injury prevention, and discharge planning
• Participate in department meetings and contribute to quality improvement initiatives

Qualifications

• Current and unrestricted Physical Therapist license (California) — required
• Minimum 2 years of clinical PT experience (outpatient and acute care experience preferred)
• BLS certification required at time of hire
• Experience with Cerner or similar electronic medical record systems preferred
• Comfort with occasional inpatient float coverage and weekend shifts
• Strong communication, documentation, and patient education skills

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor of Science in Physical Therapy or equivalent accepted

Knowledge & Skills

• Clinical expertise in outpatient physical therapy evaluation and treatment
• Familiarity with acute care principles and safe patient handling
• Proficiency with patient charting and electronic health record workflows (Cerner)
• Skilled in therapeutic exercise, manual techniques, gait training, and functional mobility
• Good problem-solving, time management, and interdisciplinary teamwork abilities
• Ability to prioritize caseloads and adapt to changing patient needs
